Death Registration – Transportation of Ashes/Mortal Remains to India

Note:

  • If the deceased is a U.S./foreign national and does not hold an OCI, we cannot process the Death Registration for carrying ashes or mortal remains to India.

Step 1) Prepare Supporting Documents & Fees

(Please fill out, Print, and sign the Miscellaneous Application, and bring it to the consulate along with the supporting documents.)

2.

Docs of Deceased (Original & Copy) - Passport and Death Certificate and Cremation Certificate

3.

Docs of Informant/Relative (Original & Copy) - Passport and Proof of Address

4.

For Ashes only:- Cremation Certificate (Original and 1 Photo Copy)

5.

For Mortal Remains only:- Non-communicable disease certificate form Health Department and Embalming Certificate and Transit Permit (All Original and 1 Photo Copy)

6.

Fees : $0 (For 1st Set of Attestation)
for every extra copy: $10 and $2 (Please issue 2 Separate Cashier’s Checks/Money Orders)

Note: Personal Checks/Cash/Credit Cards/Debit Cards are not accepted.

1. How does the Death Registration process work ?

Stage 1 – Document Submission

Follow the 2 steps mentioned above to submit the required documents to the Consulate (either in person or by mail).

Stage 2 – Consulate Processing & Issuance

The Consulate reviews the documents, verifies the application, and processes it. If the application is complete, the Death Registration will be completed and the NOC for transportation of the Ashes/Mortal Remains will be issued to the applicant on the same day.

2. Does one need to make an appointment for Death Documents Attestation ?

No, Applicants may walk into the Consulate on any working day as per below:
Timing for submitting the documents: 10:00 AM to 12:00 PM.
Timing for collection of the documents: Next day 10:00 AM - 12:00 PM

3. What is the processing time for Death Documents Attestation ?

One day (Same Day), for the application submitted in Person/over the counter.

4. Documents  valid as a proof of address/residence?

•    US Driving License OR

•    Utility Bill OR

•    Lease Agreement OR

•    State Identity Card OR

•    Mortgage Deed

5. What Documents are valid as a proof of US Visa Status?

•    Visa page of Passport (i.e. H1B, H4, L1, L2 etc.) OR

•    Green Card. OR

•    Employment Authorization Document (Work Permit) OR

•    I-797, I-140 or I-20

6. Can Applicants send the documents to the Consulate by Post or Physical Mail ?

Yes, Applicants can send the Application with Supporting Documents Via Post or Physical Mail.

Please make sure all the documents you are sending are Notarized.
