Please provide all personal details of the person (not of deceased) who is requesting for transportation of mortal remains to India.

Docs of Deceased (Original & Copy) - Passport and Death Certificate and Cremation Certificate

3.

Docs of Informant/Relative (Original & Copy) - Passport and Proof of Address

4.

For Ashes only:- Cremation Certificate (Original and 1 Photo Copy)

5.

For Mortal Remains only:- Non-communicable disease certificate form Health Department and Embalming Certificate and Transit Permit (All Original and 1 Photo Copy)

6.

Fees : $0 (For 1st Set of Attestation)
for every extra copy: $10 and $2 (Please issue 2 Separate Cashier’s Checks/Money Orders)

Please go through the checklist which is applicable to you, and bring the Application with Supporting documents to the Consulate between 10:00 AM to 12:00 PM

Applicants may walk into the Consulate on any working day as per below:

•    Utility Bill OR

•    Lease Agreement OR

•    State Identity Card OR

•    Mortgage Deed

•    Green Card. OR
