Life Certificate (For purposes other than pension)

Note:

  • Life Certificate applications for pension purposes only will be accepted directly at the Consulate.
  • For all other purposes (e.g., Life Certificate for property registration), applications must be submitted through VFS Global.

Step 1) Prepare Supporting Documents

(Please fill out, Print, and sign the Miscellaneous Application, and bring it to the consulate along with the supporting documents.)

2.

Original and Copy of: Passport and Proof of US Visa Status and Address Proof of USA

3.

Medical Examination Certificate - Original copy required

Add a recent (no later than 15 days) certificate from a registered medical practitioner, certifying that applicant (with name) has been examined (with date) by the medical practitioner and is alive on the day.

4.

Copy of the Pension book

Applicants for pension purpose may submit copy of the pension book (having name of the applicant)/bank documents of pension.

5.

Fees (For Pension purpose(s)) : $2 (Cashier's Check/Money Order)

Note: Personal Checks/Cash/Credit Cards/Debit Cards are not accepted.

1. How can one apply for Life Certificate ?

Depending on if you are (Indian Passport Holder OR USA Passport and OCI Holder OR USA Passport Holder),

Please go through the checklist which is applicable to you, and bring the Application with Supporting documents to the Consulate between 10:00 AM to 12:00 PM

2. Does one need to make an appointment for Life Certificate ?

Applicants may walk into the Consulate on any working day as per below:
Timing for submitting the documents: 10:00 AM to 12:00 PM.
Timing for collection of the documents: Next day 10:00 AM - 12:00 PM

3. What is the processing time for Life Certificate ?

One day (Next Day), for the application submitted in Person/over the counter.

4. Documents  valid as a proof of address/residence?

•    US Driving License OR

•    Utility Bill OR

•    Lease Agreement OR

•    State Identity Card OR

•    Mortgage Deed

5. What Documents are valid as a proof of US Visa Status?

•    Visa page of Passport (i.e. H1B, H4, L1, L2 etc.) OR

•    Green Card. OR

•    Employment Authorization Document (Work Permit) OR

•    I-797, I-140 or I-20

6. Can Applicants send the documents to the Consulate by Post or Physical Mail ?

Yes, Applicants can send the Application with Supporting Documents Via Post or Physical Mail.

Please make sure all the documents you are sending are Notarized.

Please send a Self Addressed prepaid envelope/Label along with your application.

7. If I am sending my application via Physical Mail - Should I send my original Documents along?

No, For Physical Mail Applications - Please send all the supporting Documents Notarized.
